Pharmacists have raised concerns over the growing impact of medication errors in Nigeria's healthcare system, identifying incorrect prescriptions, harmful drug interactions and weak collaboration among healthcare professionals as key contributors to patient harm. The concerns were expressed during the 13th Annual Peer Review Meeting of the Committee of Heads of Pharmacy in Federal Health Institutions, held in Lagos, where experts called for stronger teamwork, continuous professional training and greater use of digital health technologies to improve patient safety. They stressed that pharmacists play a vital role in identifying prescription mistakes, preventing adverse drug reactions and ensuring medicines are used safely, but noted that their expertise is not always fully utilised within healthcare facilities. President of the Pharmaceutical Society of Nigeria, Ayuba Tanko, said detecting and reporting medication errors is a core responsibility of pharmacists. He explained that effective communication among healthcare workers is essential, adding that respectful collaboration between doctors, pharmacists, nurses and other professionals can significantly reduce medication-related risks. Tanko noted that healthcare delivery depends on teamwork, warning that professional rivalry and poor communication often hinder efforts to prevent avoidable errors. Also speaking, President of the Nigeria Academy of Pharmacy, Prof. Lere Baale, described pharmacists as medication therapy experts whose responsibilities now extend beyond dispensing drugs to supporting clinical decisions, patient safety, research and healthcare innovation. According to him, the complexity of modern healthcare requires close cooperation among all professionals to achieve the best outcomes for patients. He urged pharmacists to consistently document medication errors and their interventions to demonstrate their contribution to safer healthcare delivery. Global health authorities have identified medication errors as a major cause of preventable harm, underscoring the need for stronger systems that promote safe prescribing, dispensing and administration of medicines.
